Job summary

Access Home Health in Jacksonville, FL is seeking a licensed Physical Therapist to deliver one-on-one home-based care. Full-time and PRN roles offer scheduling flexibility, independence, and strong clinical support.

You will perform evaluations, develop treatment plans, and coordinate with physicians, while ensuring accurate documentation and compassionate patient-centered care in patients' homes.

Qualifications

• Active Florida Physical Therapist License.
• Current CPR Certification.
• Strong communication and clinical decision-making skills.
• Ability to work independently.
• Patient-focused and compassionate care.
• Home health experience preferred, but not required.

Responsibilities

• Perform evaluations and develop individualized treatment plans.
• Deliver skilled physical therapy services in patients' homes.
• Monitor patient progress and modify treatment plans as needed.
• Educate patients and caregivers.
• Collaborate with physicians and interdisciplinary team members.
• Complete documentation accurately and timely in WellSky (Kinnser).

Compensation Structure

• Salary Range: 85,000 - 120,000
• Start of Care (SOC): 2.0 Units
• Evaluation / Re-Evaluation /Recertification / Resumption of Care: 1.5 Units
• Routine Visit: 1.0 Unit
• OASIS Discharge: $65

Home health offers clinicians the flexibility to build schedules that align with their personal and financial goals while maintaining a healthy work-life balance.

What to Expect

Our full-time clinicians are typically scheduled around 30 units per week, providing the opportunity for a consistent caseload and predictable earnings. We also provide a 24-unit weekly guarantee for full-time clinicians who are available to work their scheduled five-day workweek.
