Job summary

Choice Matters Home Healthcare in Jacksonville, FL is seeking a Home Health Physical Therapist (PT) to perform comprehensive evaluations and develop individualized plans of care.

You’ll help patients regain mobility and independence while working with an interdisciplinary team in a supportive, technology-enabled environment.

Qualifications

• Current Florida Physical Therapist (PT) license.
• Valid driver’s license and reliable transportation.
• Home health experience preferred, but not required.
• Experience with WellSky or another home health EMR is a plus.

Responsibilities

• Perform comprehensive physical therapy evaluations and develop individualized plans of care.
• Evaluate and treat patients to improve mobility, strength, balance, endurance, and functional independence.
• Develop evidence-based treatment plans and modify them based on patient progress.
• Educate patients and caregivers on mobility, fall prevention, home exercise programs, and safe functional movement.
• Collaborate with physicians, nurses, therapists, and other members of the interdisciplinary care team.
• Complete accurate and timely documentation while maintaining Medicare and agency standards.
