Job summary

Key Rehab is seeking a PRN Physical Therapist to support patients in the Holstein/Manson/Wall Lake, IA area within SNF settings and home health. Responsibilities include evaluating patients, planning care, and collaborating with the multidisciplinary team to achieve rehab goals.

The role also includes supervising PTAs in facilities and a weekend schedule that offers flexibility for balancing personal commitments. Active licensure in Iowa and graduation from an accredited PT program are required.

Qualifications

• Graduation from an accredited Physical Therapist program.
• Current PT license in the state of practice.
• Adherence to the Code of Ethical Conduct and state/federal regulations.

Responsibilities

• Evaluate each patient, create treatment plans, and collaborate with the interdisciplinary team to help patients reach their goals.

• Split duties between Home Health and SNF settings; weekend schedule may reduce childcare needs.

Minimum Qualifications

• Accredited Education: Graduation from an accredited Physical Therapist program.
