Job summary

Trilogy Health Services is seeking a Physical Therapist in Ohio to provide a full range of PT services, including assessment, treatment planning, and interventions in an interdisciplinary setting.

Education required is a Master Degree with 0–1 years of experience. Must maintain current PT licensure; CPR is preferred. Travel may be required, typically less than 10%.

Qualifications

• Master's degree required for PT practice.
• Strong ability to assess patients and plan individualized therapy.
• Experience with documenting patient progress per policy.

Responsibilities

• Perform comprehensive assessments and determine rehabilitation needs.
• Develop and implement individualized treatment plans including exercises and modalities.
• Educate patients and families on exercise technique and safety.
• Maintain accurate documentation of progress and outcomes.
• Collaborate with interdisciplinary team to ensure comprehensive care.

Qualifications

The Physical Therapist (PT) is responsible for providing a full range of physical therapy services including assessment, treatment planning and therapeutic interventions in an interdisciplinary environment consistent with the position’s qualifications, professional practices and ethical standards.

• Conduct thorough assessments to evaluate patients’ physical conditions, functional limitations, and rehabilitation needs.
• Develop and implement personalized treatment plans, including therapeutic exercises, manual therapy techniques, and modalities to improve patients’ physical function.
• Educate patients and their families on exercise techniques, posture, body mechanics, and strategies to manage their condition effectively.
• Maintain accurate and up-to-date documentation of patient progress, treatment plans, and outcomes in compliance with facility policies and regulatory requirements.
• Collaborate with interdisciplinary team to ensure comprehensive patient care.
• Other duties as assigned.

Education: Master Degree

Experience: 0-1 years

Licenses and Certifications

• Must have and maintain a current, valid state PT licensure or certification
• Current valid CPR certification, preferred

Travel: Yes. Minimally, as required (Typically less than 10%)
