Job summary

Qualifications

• Graduate of an accredited physical therapy program and licensed in-state.
• 1+ year post-acute care experience preferred.
• Current CPR certification.

• Strong communication, reasoning, and teamwork skills.
• Ability to lift and move patients.

Role

Provides direct physical therapy care to patients in a skilled nursing facility setting, focusing on long-term care and short-term rehabilitation.

Responsibilities

• Develop, assess, and modify treatment plans to meet patient goals.
• Supervise and evaluate therapy assistants, aides, and students.
• Maintain accurate, timely documentation and utilize therapy software.
• Collaborate with interdisciplinary teams to ensure comprehensive patient care.
• Assist in patient evacuation and deliver excellent customer service.
• Follow safety protocols, confidentiality policies, and company standards.

Qualifications & Skills

• Graduate of an accredited physical therapy program (BSPT, MSPT, or DPT); licensure in applicable state.
• Preferably 1+ year of post-acute care experience.
• Current CPR certification; proficient in Microsoft Word, Excel, and email.
• Strong communication, reasoning, and teamwork skills.
• Ability to perform physical activities including lifting and moving patients.
