Job summary

Proforma Health is seeking a licensed Physical Therapist to evaluate patients and develop individualized treatment plans aimed at restoring function, reducing pain, and promoting mobility.

You will conduct assessments, set goals, and implement evidence-based therapies while guiding therapists and staff to ensure high-quality care and regulatory compliance.

Qualifications

• Degree from an accredited Physical Therapy program.
• BLS certification within 30 days of hire.
• Strong critical thinking and ability to work independently in stressful environments.

Role

A licensed Physical Therapist responsible for evaluating patients, developing and implementing personalized treatment plans to restore function, reduce pain, prevent disability, and promote mobility.

Key Responsibilities

• Conduct comprehensive assessments, establish goals, and utilize therapeutic exercises, manual techniques, physical agents, and assistive devices.
• Educate patients on injury prevention, ergonomics, and rehabilitation strategies.
• Supervise and delegate tasks to therapy assistants, students, and support staff, ensuring compliance with clinical and regulatory standards.
• Monitor and document patient progress, adjusting plans as needed for optimal outcomes.

Qualifications & Skills

• Degree from an accredited Physical Therapy program and valid state license.
• Basic Life Support (BLS) certification within 30 days of hire.
• Strong critical thinking, decisive judgment, and ability to work independently in stressful environments.
