Job summary

The Orchards at Northwest in Detroit, MI is seeking a compassionate PRN Physical Therapist (PT) to join our in-house rehabilitation team. You will evaluate mobility and functional needs, develop individualized treatment plans, and provide therapy aimed at reducing pain and improving independence for our residents.

Qualifications

• Active Physical Therapy license in MI in good standing.
• Skilled nursing or long-term care experience preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of Net Health EDS preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs.
• Develop and implement individualized physical therapy treatment plans.
• Provide skilled therapy to reduce pain, restore function, and promote healing.
• Monitor progress and adjust treatment plans as needed; educate residents, families, and caregivers.
• Collaborate with the interdisciplinary care team.
• Maintain accurate, timely, and compliant documentation.
• Ensure services meet all professional, legal, and regulatory standards.

The Orchards at Northwest is seeking a compassionate PRN Physical Therapist (PT) to join our in-house rehabilitation team in Detroit,MI.

Key Responsibilities
