Job summary

The Orchards at Big Rapids is seeking a Part-Time/PRN Physical Therapist to join our in-house rehab team. Compensates $40/hour for full-time and $50/hour for PRN, with increases based on experience. Sign-on bonus and student loan assistance offered.

Qualifications

• SNF/LTC experience preferred.
• Current knowledge of physical therapy practices.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering physical therapy treatments to relieve pain and improve function.
• Ensure all treatment delivered follows the plan of care.
• Monitor patient response and update plan with IDT input.
• Provide education to patients, families, and caregivers.
• Collaborate with the interdisciplinary team to ensure quality care.
• Complete thorough and timely documentation compliant with regulations.
• Attend required Rehab Director meetings.

We are seeking a dedicated and compassionate PhysicalTherapist to join our healthcare team. The ideal candidate will have a strong background in patient care, with expertise in both geriatrics. This role involves working in various settings, including skilled nursing facilities and acute care environments, to help patients improve their ability to perform daily activities. The Physical Therapist will be responsible for assessing patient needs, developing individualized treatment plans, and providing therapeutic interventions to enhance the quality of life for patients.

Ideal candidates will have experience with LTC and SNF and a background using Net Health

Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Requirements:
