Job summary

The Orchards at Northwest in Detroit, MI is seeking a compassionate PRN Physical Therapist to join our in-house rehabilitation team. The role focuses on improving mobility, strength, balance, and independence for residents through personalized therapy plans in a collaborative LTC setting.

Qualifications

• Active Physical Therapy license in MI in good standing.
• Skilled nursing or long-term care experience preferred.
• Strong clinical, communication, and organizational skills.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Evaluate residents’ mobility, strength, balance, and functional needs
• Develop and implement individualized physical therapy treatment plans
• Provide skilled therapy to reduce pain, restore function, and promote healing
• Monitor resident progress and adjust treatment plans as needed
• Educate residents, families, and caregivers
• Collaborate with the interdisciplinary care team
• Maintain accurate, timely, and compliant documentation
• Ensure services meet all professional, legal, and regulatory standards
