Job summary

InHome Therapy is hiring licensed home care Physical Therapist (PT) to serve adult and geriatric patients in their homes in and around DeKalb, IL. If you are looking for a flexible schedule, consistent caseload, and a team that supports you, we would love to hear from you.

As part of our team, you will deliver in‑home physical therapy, evaluate mobility, implement rehab programs, educate patients and families, and maintain Medicare‑compliant documentation.

Qualifications

• Active physical therapy license in good standing.
• BLS CPR certification.
• Reliable transportation to see patients in their homes.

Responsibilities

• Deliver in-home physical therapy services to adult and geriatric patients in your community.
• Evaluate patient mobility, strength, and function to develop individualized treatment goals.
• Implement effective rehabilitation and therapeutic exercise programs.
• Educate patients and families on recovery plans and preventive care.
• Maintain accurate and timely home health care documentation per Medicare and agency standards.

• IHT PT Clinical Excellence Bonus - earn additional weekly pay based on visit complexity and volume
• The evaluation visit rate for this role ranges from $65 - $85 per visit

• This position is paid on a per-visit basis. Final compensation is based on experience, qualifications, and internal equity. Benefits eligibility varies based on employment status and weekly hours worked. Some full-time roles are eligible for medical benefits only; others are eligible for both medical benefits and PTO.
