Job summary

Charlottesville Health & Rehabilitation Center in Charlottesville, Virginia is seeking a caring Physical Therapist licensed in Virginia or actively pursuing licensure to join our rehab team. You will evaluate residents, develop treatment plans, and coordinate with physicians to support discharge planning.

Our in-house team includes physical, occupational, and speech therapists delivering care in a state-of-the-art rehab gym.

Qualifications

• Licensed in Virginia or graduating from an approved Physical Therapist program and actively working toward licensure.
• Excellent oral and written communication skills.
• Demonstrates effective problem-solving skills.

Responsibilities

• Evaluates residents and develops effective treatment plans.
• Obtains approval for service from referring physician.
• Treats residents according to the physician’s treatment plan and participates in discharge planning.
• Records evaluations, treatments, and charges.
• Supervises Physical Therapist Assistants and Aides.
• Screens residents for potential benefit from PT intervention.
