Job summary

Berkshire Health & Rehabilitation Center in Vinton, VA is seeking a qualified Physical Therapist (PT) to join our rehabilitation team. You will assess residents, design and implement personalized treatment plans, and collaborate with clinicians to restore function and independence.

Qualifications

  • Active Virginia license as a Physical Therapist (PT).
  • Degree in Physical Therapy from an accredited program.
  • Excellent communication and interpersonal skills.
  • Highly organized and detail oriented.
  • Ability to work independently or as part of a team.

Responsibilities

  • Evaluate residents to assess physical function, mobility, strength, balance, and rehabilitation potential.
  • Develop and implement individualized treatment plans based on each resident’s plan of care and physician orders.
  • Administer therapeutic exercises, manual therapy, gait training, and other prescribed interventions to restore function and reduce pain.
  • Supervise, coordinate, and direct the work of Physical Therapist Assistants (PTAs).
  • Explain procedures and treatments to gain residents’ cooperation and allay apprehension.
  • Monitor and document resident progress, modifying treatment plans as needed to achieve optimal outcomes.
  • Maintain residents’ medical records with thorough documentation of evaluations, treatments provided, progress notes, and response to therapy.
  • Collaborate with the interdisciplinary team to ensure continuity of care and successful achievement of rehabilitation goals.
  • Educate residents and their families on home exercise programs, safety precautions, and strategies to maintain progress after discharge.
  • Recommend appropriate assistive devices, adaptive equipment, and durable medical equipment as needed.
  • Observe emotional stability of residents, and prepare them for continuing care after discharge.
  • Perform other duties as required to provide exceptional care to each resident.

What You Need
