About the Role:

As a Per Diem Physical Therapist (PT), you will work part-time, as needed, to assess, plan, and implement interventions for our residents. Physical Therapist (PT) will also maintain accurate and up-to-date records of treatment plans, progress, and outcomes.

Physical Therapist (PT) Responsibilities:

• Conduct thorough assessments of residents' physical abilities and develop individualized treatment plans
• Implement and adjust treatment plans to achieve desired outcomes, ensuring residents' safety and comfort
• Provide education and training to residents, family members, and caregivers on physical therapy techniques and exercises
• Collaborate with other members of the therapy team, including occupational and speech therapists, to ensure comprehensive care
