Aspire Senior Living Carthage is seeking a skilled Physical Therapist (PT) to join our team. This per diem/PRN role offers flexible scheduling and the opportunity to improve residents' mobility and independence in a warm, supportive environment.

Responsibilities include initial evaluations, ongoing treatments, developing and adjusting individualized plans, collaborating with physicians and nurses, using diverse PT techniques, and maintaining accurate treatment records.

Responsibilities

• Develop and implement individualized physical therapy treatment plans for residents with a wide range of medical conditions and functional abilities
• Conduct initial evaluations, reassessments, and ongoing treatments to achieve residents' goals and improve their overall health and well-being
• Collaborate with other healthcare professionals, including physicians, nurses, and occupational therapists, to ensure comprehensive care and effective communication
• Utilize a variety of physical therapy techniques, modalities, and equipment to promote residents' independence, mobility, and safety
• Maintain accurate and detailed records of treatments, progress, and outcomes
