Kadima Rehabilitation & Nursing at Latrobe SNF/LTC in Latrobe, PA is seeking a full-time Physical Therapist to join our in-house rehab team. You will evaluate and treat residents, promote recovery, and support a family-friendly community focused on excellence.

The role emphasizes delivering care per plan, documenting outcomes, and collaborating with caregivers. Experience in SNF/long-term care and familiarity with Net Health EHR are preferred, as is the ability to adapt to diverse resident needs.

Qualifications

• SNF/Long term care experience preferred.
• Current knowledge of physical therapy treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Responsabilités

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assure all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
• Measures & records patient's motor function, strength, and muscle performance
• Monitors patient responses to treatments and makes modifications during treatment sessions as necessary
• Completes documentation in accordance with company policy and state/federal regulations

The Physical Therapist (PT) plays a pivotal role in the rehab department at our skilled nursing facility, promoting the physical well-being and functional recovery of our residents.

Duties/Responsibilities:

• Evaluate, plan, and administer treatment to residents and patients with temporary or long-term disabilities to relieve pain, restore or improve function, and promote healing.
• Assured all treatment is delivered in accordance with an established plan of care
• Provide clinical support and instruct patients, families, and caregivers
