Pay Range: $63,000 - $114,000

Job Description:

A Physical Therapist (PT) will implement treatment programs to assist pediatric patients with physical, neurological, cognitive, and social/emotional disabilities or delays by planning and administering physical therapy services in the home and community.

Responsibilities:

1. Provide high-quality care by performing evaluations, interpreting assessment results, developing goals appropriate to the child, and creating and implementing physical therapy treatment plans in conjunction with the physician.
2. Assist pediatric patients in developing or regaining physical, neurological, cognitive, and/or social/emotional functioning to improve their independence by implementing skilled interventions.
3. Educate, coach, and empower caregivers and families to develop skills for carrying over therapeutic activities into daily routines.
4. Coordinate with referral partners to provide services according to physician orders.
5. Ensure continuation of therapeutic plans post-discharge by designing and instructing patients, families, and caregivers in home exercise programs.
6. Document patient care services and care coordination in an electronic medical record system.
7. Maintain patient confidentiality by keeping information secure.

Requirements:

1. State license
