Job summary

Continuum Therapy Partners, LLC is seeking a Physical Therapist for PRN coverage in Yorkville, IL. You’ll deliver high-quality, patient-centered care with consistent standards alongside the full-time team.

The ideal candidate holds a DPT, active IL PT license, NPTE, and prefers at least 1 year of experience. You’ll evaluate, plan, and adjust treatments, educate patients and families, and document outcomes in a collaborative setting.

Qualifications

• Current, unencumbered IL PT license; must pass NPTE
• 1 year of PT experience preferred
• Evaluates, develops, and implements treatment plans in coordination with other disciplines
• Documents patient progress and adjusts plans of care based on outcomes

Physical Therapist Requirements & Responsibilities

• Doctor of Physical Therapy (DPT) degree required
• Current, unencumbered state Physical Therapy License; must pass the National Physical Therapy Examination (NPTE)
• 1 year of previous experience as a Physical Therapist preferred
