Job summary

LiveWell Homecare Agency, LLC seeks a dedicated physical therapist in Marietta, GA to assess, plan, and participate in rehab programs that improve mobility and reduce pain for home health patients.

You will collaborate with physicians on goals and discharge plans, document evaluations and treatments, educate patients and families, and uphold ethical standards, confidentiality, and teamwork. You will participate in onboarding and ongoing trainings to maintain high-quality home health care.

Qualifications

• Doctorate, Masters, or Bachelors in Physical Therapy.
• Licensure as a Physical Therapist and BLS certification.
• Effective communication, customer service orientation, teamwork, and basic computer skills.

Responsibilities

• Implements treatment plans, selects activities, revises goals with patient input, provides discharge instructions and referrals.
• Collaborates with physicians to establish and evaluate treatment programs, goals, and discharge plans, involving patients and families in care planning.
• Records evaluations, treatment sessions, and family education according to departmental protocols.
• Upholds ethical practices, maintains confidentiality, punctuality, and effective communication. Participates in meetings, training, and quality improvement activities.
• Demonstrates a patient-focused approach, fosters teamwork, and applies service recovery principles to ensure positive experiences.
• Engages in ongoing education, completes required competencies, and participates in onboarding and system training.
