Qualifications

• Masters in Physical Therapy (Doctorate preferred).
• Licensure as a Physical Therapist and BLS certification.
• Minimum one year of experience in rehab, acute care, or SNF settings.
• Effective communication, customer service focus, team orientation.
• Basic computer skills.

Responsibilities

• Collaborate with physicians to establish and evaluate treatment plans, goals, and discharge strategies, incorporating patient and family input.
• Implement therapy plans, select appropriate treatments, adjust goals as needed, and provide patient education, follow-up, and community referrals.
• Document evaluations, treatment sessions, and discharge instructions in accordance with departmental protocols.
• Uphold ethical standards, maintain confidentiality, punctuality, and dress code, communicate effectively, attend meetings, and ensure safe, efficient use of materials and equipment.
• Deliver respectful, compassionate care, foster teamwork, and demonstrate service recovery principles.
• Engage in ongoing education, complete required competencies, participate in orientation, and adhere to mandatory training requirements.
