Job summary

Fruitvale Healthcare Center is seeking a qualified Physical Therapist to evaluate and treat residents within scope of practice and per federal guidelines. The role emphasizes patient-centered care and adherence to the plan of care.

Applicants must hold a CA PT license and an approved degree in Physical Therapy; new graduates are welcome. A competitive salary range with per diem/on-call duties, PTO, and referral bonuses is offered.

Qualifications

• Bachelor, Master, or Doctorate in Physical Therapy.
• Current license to practice Physical Therapy in California.
• 1 year clinical experience preferred; new graduates welcome.

Responsibilities

• Evaluate and administer treatment to residents with neuromuscular and musculoskeletal issues.
• Perform tests and measurements for strength, ROM, coordination, and vital functions.
• Provide direct resident care per established plan of care.
• Maintain confidentiality of patient records.
• Conduct home assessments to ensure safe discharge environments.
• Complete forms and documentation per policy and regulations.

Physical Therapist (PT) Duties and Responsibilities within scope of practice and federal guidelines:

• Evaluates, plans and administers treatment to patients with problems related to neuromuscular and musculoskeletal systems.
• Administers and interprets tests and measurements for muscle strength, coordination, range of motion, and respiratory and circulatory efficiency.
• Provides direct resident treatment in accordance with an established plan of care.
• Maintains confidentiality of patient records.
• Conducts home assessments simulated or actual, as appropriate, to ensure discharge of residents to a safe environment.
• Completes required forms and documents in accordance with company policy and state and/or federal regulations.

Physical Therapist (PT) Qualifications and Skills:

• Bachelor of Science, Master of Science, or Doctorate degree in Physical Therapy.
• One (1) year of clinical experience preferred. New graduates welcomed!
• Current license to practice Physical Therapy issued by the state of California.
