Job summary

IHA in New Hampton, IA seeks a part-time Physical Therapist to provide client-centered care in home settings, developing individualized treatment plans within a multidisciplinary team.

You will perform initial assessments (OASIS where applicable), create goals, and coordinate with physicians to update the plan of care, fostering strong collaboration with care teams.

Qualifications

• Graduate of a university approved by APTA.
• Licensed in Iowa to practice physical therapy.
• Minimum of 1-year experience preferred.
• Community or home health experience desirable.

Responsibilities

• Evaluates clients to obtain and interpret data necessary for treatment planning and implementation, in the client's home setting.
• Performs initial assessment/start of care services including OASIS assessment for Physical Therapy (PT) clients when applicable.

• Establishes patient-centered goals and collaborates with other members of the interdisciplinary team as indicated.

Description

This individual works collaboratively within a multidisciplinary team to develop an individualized physical therapy treatment plan for client's home/residence.
