Job summary

MercyOne Des Moines Home Care is seeking a part-time Physical Therapist to provide home-based therapy in Des Moines, IA. You will deliver evidence-based care, develop personalized rehab plans, and support clients in regaining independence within their homes.

Qualifications

• Degree from an APTA-accredited Physical Therapy program.
• Active state PT license required.
• 1+ year experience or 6–8 week preceptorship in PT.
• Home health experience preferred.
• Valid driver’s license and reliable transportation.
• Strong organizational and time-management skills.

Responsibilities

• Conduct comprehensive assessments and develop individualized treatment plans.
• Deliver therapeutic interventions (manual therapy, modalities, exercise).
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise PTAs and HHAs (as permitted).
• Collaborate with interdisciplinary teams and participate in care conferences.
• Monitor progress and adjust treatment plans as needed.
• Maintain accurate, timely documentation.

Your Role

As a Physical Therapist, You Will

• Provide professional, evidence-based care in accordance with physician orders and agency policies.
• Help clients regain independence and improve quality of life through personalized rehabilitation plans.
