Job summary

MercyOne Des Moines Home Care is seeking a Physical Therapist to join a compassionate team delivering in-home therapy. This part-time PRN role offers 0–20 hours per week in Des Moines, IA, with travel to patient homes and Trinity Health locations as needed.

You will perform assessments, develop individualized treatment plans, deliver evidence-based interventions, educate families, supervise PTAs, and collaborate with an interdisciplinary team while maintaining thorough documentation.

Qualifications

• Degree from an APTA-accredited Physical Therapy program.
• Active state PT license.
• 1+ year experience or completion of a 6–8 week preceptorship.
• Home health experience preferred.

Responsibilities

• Provide professional, evidence-based care in line with physician orders and policies.
• Develop individualized treatment plans and comprehensive assessments.
• Deliver interventions (manual therapy, modalities, exercise programs).
• Educate clients, families, and caregivers on therapy techniques and home programs.
• Supervise PTAs and Home Health Aides as allowed.
• Collaborate with interdisciplinary teams and participate in care conferences.
• Monitor progress and adjust treatment plans; maintain documentation.

Your Role

As a Physical Therapist, you will:

• Provide professional, evidence-based care in accordance with physician orders and agency policies.
• Help clients regain independence and improve quality of life through personalized rehabilitation plans.

Key Responsibilities
