Job summary

LiveWell Homecare Agency is seeking a Physical Therapist to assess, plan, and participate in rehabilitative programs that improve mobility, reduce pain, and prevent deformity for patients with disease or injury.

You will implement and revise treatment plans, conduct assessments, set goals, and document progress while collaborating with families and physicians to ensure quality care.

Qualifications

• Bachelor’s, Master’s, or Doctorate in Physical Therapy.
• Licenses and certifications: PT license and BLS certification required upon hire.
• Effective communication, customer service orientation, teamwork, and basic computer skills.
