Gig Harbor Health & Rehabilitation in Gig Harbor, WA is seeking a compassionate Physical Therapist to join our in-house therapy team at a 120-bed skilled nursing facility. You will provide direct patient care, perform PT assessments, and collaborate with a multidisciplinary team in a residential setting.

Qualifications

• Graduated from an accredited Physical Therapy program.
• Valid Physical Therapy license for the state of practice (WA).
• Perform PT assessments of referred patients and provide direct patient care.
• CPR Certification required.
• Experience with PCC EMR is a plus.

Responsabilités

• Assess patients referred for physical therapy and develop treatment plans.
• Document patient progress and plan in PCC EMR.
• Collaborate with care teams to optimize rehabilitation outcomes.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
