Job summary

Alderwood Post Acute & Rehab in Lynnwood, WA is seeking a compassionate Physical Therapist to join our in-house therapy team on a PRN basis for 20 hours per week across all days. We offer an opportunity to contribute to high-quality care at a 113-bed skilled nursing facility with a supportive environment.

Qualifications

• Graduated from an accredited Physical Therapy program (PT).
• Valid PT license for the state of practice.
• Conduct PT assessments of referred patients with direct patient care.
• CPR Certification.
• Experience with EMR documentation (PCC preferred).

Responsibilities

• Provide direct patient care and PT evaluations as part of the in-house therapy team.
• Collaborate with clinicians to develop and adjust treatment plans.
• Document patient progress and maintain accurate EMR records.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.
