Job summary

Lacey Post Acute & Rehab, a 120-bed skilled nursing facility in Lacey, WA, is seeking a compassionate Physical Therapist to join our in-house therapy team. You will provide direct patient care, conduct PT assessments, and collaborate with a motivated team to foster recovery and improve functional outcomes.

Qualifications include graduation from an accredited PT program, a valid PT license for WA, and CPR certification.

Qualifications

• Graduated from an accredited Physical Therapy (PT) college or university.
• Valid Physical Therapy (PT) license for state practicing.
• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.
• Proficient in EMR documentation (we use PCC but experience in this system is not required).
• CPR Certification.

Responsibilities

• Provide direct physical therapy to referred patients.
• Document patient progress and outcomes in the EMR.
• Collaborate with the therapy team to optimize rehabilitation plans.
