Birch Creek Post Acute & Rehabilitation in Tacoma, WA is seeking a compassionate Physical Therapist to join our in-house therapy team. You will provide direct patient care, perform assessments, and collaborate with an experienced healthcare team in a 124-bed skilled nursing facility.

Qualifications

• Graduate from an accredited Physical Therapy program and obtain licensure.
• Licensed PT in Washington state.
• Experience conducting PT assessments and providing direct patient care.
• Proficient in EMR documentation (PCC system).
• CPR certification is required.

Responsibilities

• Provide direct patient care and PT assessments for referred patients.
• Collaborate with a multidisciplinary team to plan and implement therapy plans.
• Document progress and outcomes in the PCC EMR system.

Birch Creek Post Acute & Rehabilitation offers around-the-clock skilled nursing in a residential setting. Our community features an outdoor recreation area, a patio garden, a rehab gym, a beauty/barber shop, a library, pastoral support, and transportation services.Large 1000 sqft gym space at south end of building with lots of windows/natural light, ADL Bathroom and Kitchen/Laundry training spaces, OmniVR: 3-D virtual reality system used for strengthening, balance, endurance, coordination, and cognitive skills development.

Qualifications:

• Graduated from an accredited Physical Therapy (PT) college or university.

• Valid Physical Therapy (PT) license for state practicing.

• Conduct Physical Therapy assessments of referred patients providing direct patient care to assess medical conditions, functional capabilities, limitations, and potential for rehabilitation.

• Proficient in EMR documentation (we use PCC but experience in this system is not required).

• CPR Certification.
