Job summary

Lake Park of Madison Nursing and Rehabilitation Center in Madison, FL seeks a licensed Physical Therapist (PRN) to join our team providing PT services in a skilled nursing facility. You will work with an interdisciplinary team to develop and implement effective treatment plans to help patients reach their goals.

You will evaluate and treat patients with orthopedic, neurological and cardiopulmonary needs, maintain progress notes, and participate in care conferences.

Qualifications

• Florida licensed Physical Therapist in good standing.
• Minimum 0 years of experience in skilled nursing facility (SNF) setting.
• Strong communication and interpersonal skills.

Responsibilities

• Evaluate and treat patients with PT needs in a skilled nursing facility.
• Develop and implement individualized treatment plans to meet patient goals.
• Conduct physical therapy treatments, including exercises, modalities and other interventions.
• Communicate effectively with patients, families and the healthcare team to ensure seamless care.
• Maintain accurate documentation of patient progress and treatment plans.
• Participate in patient care conferences and contribute to the treatment plan.

About the Role:

We are seeking a highly motivated and experienced Physical Therapist (PRN) to join our dynamic team. As a PRN, you will provide physical therapy services to our patients in a skilled nursing facility setting. You will work closely with our interdisciplinary team to develop and implement effective treatment plans to help our patients achieve their goals.

Responsibilities:

• Evaluate and treat patients with a variety of physical therapy needs, including orthopedic, neurological, and cardiopulmonary conditions
• Develop and implement individualized treatment plans to achieve patient goals
• Conduct physical therapy treatments, including exercises, modalities, and other interventions
• Communicate effectively with patients, families, and the healthcare team to ensure seamless care
• Maintain accurate and timely documentation of patient progress and treatment plans
• Participate in patient care conferences and contributes to the development of the patient's treatment plan

Requirements:

• Currently licensed as a Physical Therapist in the state of Florida
