Magnolia Gardens Center for Nursing and Rehabilitation is seeking a licensed Physical Therapist (PRN) to evaluate, plan, and deliver rehabilitative services to residents in our skilled nursing setting. • Graduate of an accredited Physical Therapy program.
• Current state licensure or eligibility to obtain one.
• Experience in a skilled nursing or long-term care setting preferred but not required.
• Strong clinical judgment, interpersonal skills, and a collaborative mindset.
• Lift up to 50 lbs and assist with patient mobility.

• Evaluate residents and develop individualized treatment plans based on physician orders.
• Deliver PT treatments and adjust plans to meet functional goals.
• Supervise PT assistants, rehab techs, and students as required.
• Maintain accurate documentation per regulatory standards.
• Collaborate with nurses, physicians, and occupational therapists.
• Educate residents, families, and staff on therapy techniques and home programs.
• Participate in discharge planning and quality improvement initiatives.
• Travel or float to nearby facilities as needed.

We are seeking a licensed and motivated Physical Therapist (PT) to evaluate, plan, and deliver rehabilitative services to residents recovering from injury, illness, or surgery.
