Job summary

Nexus at Elmwood Park in Illinois is seeking a PRN Physical Therapist to support patient rehabilitation in a skilled nursing care environment.

You will assess evaluations, develop and implement treatment plans, and collaborate with the care team to promote healing and independence for residents.

This role requires thorough documentation and compliance with all state/federal regulations, with emphasis on patient education and teamwork.

Qualifications

• Active/Valid Illinois license in Physical Therapy.
• SNF/Long term care experience preferred.
• Current knowledge of treatment practices.
• Knowledge of Net Health Electronic Documentation System preferred.

Responsibilities

• Assess patient evaluations and test results.
• Develop treatment plans with a variety of techniques.
• Create personalized rehabilitation programs for patients.
• Administer PT treatments to relieve pain and restore function.
• Provide clinical support and education to patients and families.
• Collaborate with the interdisciplinary team to ensure quality care.
• Ensure documentation complies with regulatory requirements.

PRN Physical Therapist Responsibilities:

• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.

PRN Physical Therapist Qualifications:
