Job summary

Pro Rehab in Eagle, Colorado is seeking a licensed Physical Therapist to join Castle Peak, a Cassia community, on a PRN basis. Starting pay is $60 per hour with responsibilities spanning assessment, treatment planning, hands-on therapy, and patient education at our Eagle location.

The role requires current CO PT licensure, graduation from an accredited PT program, and strong knowledge of Home Health care regulations. English communication skills are essential.

Qualifications

• Must be a graduate from an Accredited Physical Therapy Program and have a bachelor's degree.
• Must possess a current accredited CO Physical Therapist License.
• Must possess strong knowledge regarding state, federal, and local regulations as they pertain to Home Health care.
• Must be able to communicate in English, both verbally and in writing and possess sufficient communication skills to perform the tasks required.

Responsibilities

• Assess and diagnose patients to develop personalized treatment plans.
• Develop and implement rehabilitation programs to improve functional deficits.
• Provide hands-on therapy including manual therapy and therapeutic exercises.
• Educate patients and families about exercises, injury prevention, and pain management techniques.
• Monitor patient progress and adjust treatment plans as needed.
• Collaborate with doctors, nurses, and other therapists to ensure comprehensive care.

Starting Pay: $60 / hour

Physical Therapist Responsibilities:

• Assess & Diagnose: Evaluate patients' physical conditions, movement abilities, and medical history to develop personalized treatment plans.
• Develop & Implement Treatment Plans: Create and execute rehabilitation programs to improve the functional deficits of the patient.
• Provide Hands-on Therapy: Perform manual therapy, therapeutic exercises, and use modalities such as but not limited to ultrasound and electrical stimulation as needed.
• Educate & Support Patients: Provide patient, caregiver, staff education as deemed clinically necessary such as but not limited to: pain management techniques and fall risk safety. Teach patients and families about exercises, injury prevention, and pain management techniques.
• Monitor Progress & Adjust Plans: Track patient outcomes, document progress, and modify treatment plans as needed.
• Collaborate with Healthcare Teams: Work with doctors, nurses, and other therapists to ensure comprehensive patient care.

Physical Therapist Qualifications:

• Must be a graduate from an Accredited Physical Therapy Program and have a bachelor's degree
