We are hiring for a PRN Physical Therapist.

Salary Range: $45.00-$49/hour based on per visit pay model.

Responsibilities

The Physical Therapist (PT, DPT, RPT) in Home Health is responsible for assessing and evaluating patient care needs related to functional status/mobility, endurance/stamina, pain, and other medical conditions. Based on this assessment, the Physical Therapist helps determine a treatment plan, performs interventions to improve and enhance the patient's well-being, and evaluates the patient's progress.

• Evaluate the patient's functional status and physical therapy needs. Assist the physician in developing the therapy plan of care.
• Observe and report the patient's response to treatment and any changes in condition to the supervising nurse and/or physician.
• Treat patients to relieve pain, develop and restore function, and/or maintain maximum functioning using accepted standards of practice.
• Instruct and inform team members, patients, and families/caregivers regarding the plan of care, functional limitations, and progress toward goals.

Education and Experience

Licensure Requirements

• Current Physical Therapy licensure in the relevant state.
• Current CPR certification.
• Must have a valid driver's license, vehicle insurance, and access to a dependable vehicle.
