Job summary

Vista Ridge Senior Care in Lakeway, TX is hiring a licensed Physical Therapist to develop and implement individualized plans of care for residents and short-term patients. Full-time position with a focus on high-quality patient care and collaboration with an in-house therapy team.

You will supervise PT Assistants, participate in screenings, document outcomes, maintain treatment spaces, and train patients and families on follow-up care.

Qualifications

• Current Texas licensure as a PT is required.

Responsibilities

• Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
• Provide supervision for staff Physical Therapy Assistants in delivery of the PT plans of care.
• Assist as needed with patient screenings for the need for Physical Therapy.
• Communicate progress and any barriers toward PT goals with interdisciplinary team members.
• Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
• Assist with maintaining rehab gym equipment and treatment area in a safe manner, using infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each PT patient’s specific plan of care and recommendations for follow up.

This position is for a skilled nursing facility in Lakeway, which has both short-term and long-term patients. Vista Ridge Senior Care has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients.
