Job summary

Peach Tree Place in Weatherford, TX is seeking a dedicated Physical Therapist to assess, plan, and administer rehabilitation programs designed to reduce pain and restore function. You will work with an interdisciplinary team to ensure high-quality patient care and complete timely documentation.

New grads are welcome. Competitive compensation and flexible schedules are offered, with a focus on patient-centered care in a SNF/long-term care setting.

Qualifications

• Active/Valid Texas license in Physical Therapy.
• In good standing with regulatory agencies and licensing boards.

Responsibilities

• Consulting with patients to learn about their physical condition and goals.
• Assessing and interpreting patient evaluations and test results.
• Developing and implementing treatment plans using varied techniques.
• Creating personalized rehabilitation programs to restore function and reduce pain.
• Administering prescribed physical therapy treatments per plan of care.
• Monitor patient responses and adjust treatment with IDT collaboration.
• Provide education to patients, families, and caregivers.
• Collaborate with interdisciplinary team to ensure quality care.
• Ensure documentation complies with regulatory requirements.
• Attend required meetings designated by Director of Rehab.

Job description

Physical Therapist Responsibilities:

• Consulting with patients to learn about their physical condition.
• Assessing and interpreting patient evaluations and test results.
• Developing treatment plans using a variety of treatment techniques.
• Creating personalized rehabilitation programs for patients.
• Administering medically prescribed physical therapy treatments to relieve pain, improve or restore function, and promote healing.
• Ensure all treatment delivered is in accordance with the established plan of care.
• Monitor patient response to treatment intervention and modify treatment approach and directive as clinically appropriate with IDT collaboration.
• Provide clinical support and education to patients, family members, and caregivers.
• Collaborate with other members of the interdisciplinary team to ensure quality patient care is provided.
• Ensure documentation is completed thoroughly and timely and complies with all company and state/federal regulatory requirements.
• Attend required meetings as designated by the Director of Rehab.

Physical Therapist Skills:

• SNF/Long term care experience preferred.
• Current knowledge of treatment practices
• Knowledge of Net Health Electronic Documentation System preferred.
• Ability to manage patients with different types of personalities.

Physical Therapist Requirements:

• Active/Valid Texas license in Physical Therapy
• In good standing with all regulatory agencies and licensing boards
