Job summary

Harbor Point Skilled Nursing is seeking a licensed Physical Therapist to develop and implement plans of care for residents and short-term patients, with supervision of PT Assistants and collaboration with the interdisciplinary team.

The role requires Texas PT licensure; new grads welcome. Full-time, on-site position with benefits including health, dental, vision, life insurance, and CEU/licensure reimbursement.

Responsibilities

• Develop and implement the Physical Therapy plan of care for facility residents and short-term patients.
• Provide supervision, as needed, for staff Physical Therapy Assistants in delivery of the PT plans of care.
• Assist as needed with patient screenings for the need for Physical Therapy.
• Communicate progress and any barriers toward PT goals with interdisciplinary team members.
• Complete timely PT documentation in compliance with regulatory, licensing, and payor requirements.
• Assist with maintaining both the rehab gym equipment and treatment area in a safe manner, using good infection prevention methods.
• Provide and document patient, family, and caregiver training regarding each PT patient's specific plan of care and recommendations for follow-up.

This position is for a skilled nursing facility with both short-term and long-term patients. Harbor Point Skilled Nursing has an in-house therapy team with smaller daily case loads, all new equipment, and an emphasis on optimizing time with patients.
