Qualifications

• Current, valid California PT license in good standing.
• Experience in SNF or geriatric/post-acute settings preferred.
• Excellent communication and clinical reasoning skills.

Responsibilities

• Perform comprehensive PT evaluations and develop treatment plans.
• Implement evidence-based interventions to improve mobility and independence.
• Collaborate with IDT including nurses, therapists, and physicians.
• Document accurately per state and federal regulations.
• Educate residents and families on therapy programs and safety.
