Job summary

Salinas Valley Post Acute is seeking an experienced Assistant Director of Rehabilitation (Physical Therapist) to join our leadership team. This full-time role blends clinical excellence with departmental oversight to ensure high-quality therapy services and resident outcomes.

The position requires graduation from an accredited Physical Therapy program and a California PT license, with preferred leadership experience in a skilled nursing or post-acute setting.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current and unrestricted Physical Therapist (PT) license in California.
• Experience in skilled nursing facility, post-acute care, rehab center, or long-term care preferred.
• Leadership or supervisory experience preferred.
• Strong clinical assessment, documentation, and care planning skills.
• Excellent communication, organization, and interpersonal abilities.
• Ability to work effectively within an interdisciplinary team.

Responsibilities

• Evaluate residents to determine PT needs and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Supervise PTAs, rehab aides, and students.
• Communicate with physicians, nursing staff, residents and families about progress and discharge planning.
• Participate in care planning meetings and interdisciplinary activities.
• Provide in-service education and training for facility staff.
• Maintain accurate, compliant clinical documentation.
• Educate residents and families on home programs and fall prevention.
