Job summary

Citrus Heights Post Acute in Citrus Heights, CA is seeking a Physical Therapist (PT) to join our rehabilitation team. The role offers a competitive hourly pay of $65.00–$70.00 based on experience, and a full-time schedule in a skilled nursing/post-acute setting.

You will evaluate residents, develop treatment plans, supervise PTAs, and collaborate with an interdisciplinary team to maximize residents’ mobility and independence. A CA PT license and CPR may be required.

Qualifications

• Current California Physical Therapist (PT) license in good standing.
• Bachelor's degree in Physical Therapy (Master's or Doctorate preferred).
• Strong communication and interpersonal skills.
• Ability to work collaboratively with residents, families, physicians, and interdisciplinary team members.
• Current CPR certification (if required by licensure).

Responsibilities

• Evaluate residents to determine therapy needs, functional limitations, and rehabilitation potential.
• Develop, implement, and update individualized treatment plans.
• Improve residents' mobility, strength, endurance, balance, and overall functional independence.
• Supervise and direct Physical Therapist Assistants (PTAs) in accordance with state regulations.
• Assist with training and development of Restorative Aides.
• Collaborate with physicians, nursing staff, families, and interdisciplinary team members regarding resident progress and discharge planning.
• Participate in care conferences, utilization review meetings, and rehabilitation team meetings.
• Provide educational in-services and training to facility staff.
• Complete accurate and compliant documentation in accordance with facility, state, federal, and payer requirements.
• Educate residents and families on home exercise programs, safety techniques, and fall prevention strategies.
• Recommend and coordinate durable medical equipment to support resident independence and mobility.
