Job summary

KeystoneCare is seeking a qualified Physical Therapist to provide in-home rehabilitation services. You will assess and treat patients in their homes, planning programs to improve mobility and independence, collaborating with a multidisciplinary team.

Requirements include PA PT license, 2+ years in home health or similar settings, and a valid driver's license. Flexible scheduling and travel between sites are offered to support patient care.

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in home health or similar care setting.
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting.
• Experience providing care in hospice-adjacent contexts.
• Strong communication skills for effective collaboration with patients, families, and the care team.
