KeystoneCare is seeking a Physical Therapist to provide personalized rehabilitation services in patients' homes. This role requires a compassionate professional dedicated to helping patients regain mobility and independence.

You will assess and carry out treatment plans while collaborating with a multidisciplinary team. If you have a PA Physical Therapy License and experience with geriatric patients, we encourage you to apply.

Responsibilities

• Provide personalized rehabilitation services to patients in their homes


• Assess, plan, and carry out treatment programs focused on improving mobility, strength, and overall functional abilities


• Collaborate closely with a multidisciplinary team to support patient care and recovery


• Guide patients through their recovery journey with compassion and professionalism


• Help restore patients' independence and well-being through high-quality therapeutic care

Requirements

• Graduate of an accredited Physical Therapy program


• Current Pennsylvania Physical Therapy License


• Minimum two years of physical therapy experience in Home Health or similar settings


• Experience treating geriatric patients in a home health or similar care setting


• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting


• Experience providing care in hospice-adjacent contexts


• Valid Driver's License and reliable vehicle


• Proof of Auto Insurance


• Strong communication skills for effective collaboration with patients, families, and the care team


• Excellent organizational abilities and experience managing a diverse caseload


• Eligible to work in the United States


• Experience with Matrix Care is a plus
