Job summary

KeystoneCare is seeking a Physical Therapist to deliver in-home rehabilitation, focusing on mobility, strength, and independence for patients in their homes.

Candidates should be licensed in Pennsylvania, with at least two years of home health experience and experience with geriatric, post-surgical, neurological, and orthopedic rehab. A valid driver’s license and reliable vehicle are required.

Qualifications

• PA Physical Therapy License required.
• Minimum 2 years of physical therapy experience in Home Health or similar settings.
• Experience with geriatric patients in home health or similar care setting.
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting.
• Eligible to work in the United States.

Responsibilities

• Graduate of an accredited Physical Therapy program

• Current Pennsylvania Physical Therapy License
• Minimum two years of physical therapy experience in Home Health or similar settings
• Experience treating geriatric patients in a home health or similar care setting
• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting
• Experience providing care in hospice-adjacent contexts
• Valid Driver’s License and reliable vehicle
• Proof of Auto Insurance
• Strong communication skills for effective collaboration with patients, families, and the care team
• Excellent organizational abilities and experience managing a diverse caseload
• Eligible to work in the United States
• Experience with Matrix Care is a plus.
