Job summary

KeystoneCare Home Health and Hospice is seeking a dedicated Physical Therapist to provide in-home rehabilitation, helping patients regain mobility, strength, and independence with compassionate, one-on-one care.

You will collaborate with a multidisciplinary team to plan and implement treatment for geriatric patients, including post-surgical, neurological, and orthopedic rehab in home settings. A PA license and valid driver's license are required, and travel between sites is expected.

As a Physical Therapist at KeystoneCare, you will make a meaningful impact by helping patients strengthen mobility, restore function, and maintain independence through compassionate in-home care. Responsibilities

Requirements

• Graduate of an accredited Physical Therapy program


• Current Pennsylvania Physical Therapy License


• Minimum two years of physical therapy experience in Home Health or similar settings


• Experience treating geriatric patients in a home health or similar care setting


• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting


• Experience providing care in hospice-adjacent contexts


• Valid Driver's License and reliable vehicle


• Proof of Auto Insurance


• Strong communication skills for effective collaboration with patients, families, and the care team


• Excellent organizational abilities and experience managing a diverse caseload


• Eligible to work in the United States


• Experience with Matrix Care is a plus
