Job Details

The Physical Therapist is responsible for the evaluation, treatment, and discharge planning of all patients referred for their services. The Physical Therapist plays an integral role in healthcare, aiming to help patients reach their highest level of function. The therapist will uphold the mission, vision, and values of Jefferson Health.

They work in conjunction with other healthcare professionals to coordinate treatment plans and care.

Essential Functions:

  1. Provide therapy evaluations, treatment planning, interventions, and discharge planning for a full patient caseload across acute care, skilled facilities, acute rehab, and outpatient settings as appropriate.
  2. Plan, organize, and deliver treatment programs to help clients/patients recover and rehabilitate.
  3. Utilize tools and techniques to administer safe therapeutic interventions and training for patients and families.
  4. Record and document clients'/patients' health progress, including evaluations, interventions, daily treatments, progress notes, and discharge summaries.
  5. Perform other duties as assigned.

Job Requirements:

  • Bachelor's, Master's, or Doctoral Degree from an accredited Physical Therapy Program.
  • 0-2 years of clinical practice required; 1 year preferred.
  • Valid state license in Physical Therapy.
  • BLS and CPR certification required via AHA.
