Job summary

KeystoneCare, a nonprofit provider of in-home healthcare services, is looking for a compassionate Physical Therapist to provide personalized rehabilitation services. In this role, you will help patients regain mobility and maintain independence through one-on-one support in their homes.

The ideal candidate will have a valid Pennsylvania Physical Therapy License, at least two years of physical therapy experience, and a commitment to enhancing patient recovery and well-being through compassionate

Qualifications

• Graduate of an accredited Physical Therapy program.
• Current Pennsylvania Physical Therapy License.
• Minimum two years of physical therapy experience in Home Health or similar settings.
• Experience treating geriatric patients in a home health or similar care setting.
• Experience with post-surgical rehabilitation and orthopedic rehabilitation.
• Valid Driver’s License and reliable vehicle.
• Strong communication skills for effective collaboration.

• Experience with post-surgical rehabilitation, neurological conditions, and orthopedic rehabilitation in a home health or similar care setting
• Experience providing care in hospice-adjacent contexts
• Valid Driver’s License and reliable vehicle
• Proof of Auto Insurance
• Strong communication skills for effective collaboration with patients, families, and the care team
• Excellent organizational abilities and experience managing a diverse caseload
• Eligible to work in the United States
• Experience with Matrix Care is a plus.
