Gardens of Mayfield Heights is seeking a PRN Physical Therapist (PT) to join our in-house therapy team. This flexible opportunity offers competitive PRN compensation and no set schedule, with the chance to make a meaningful difference in residents' lives.

As a PT on our in-house team, you will evaluate residents, develop individualized plans, and collaborate with PTAs and the interdisciplinary team to support mobility and independence in a long-term care setting.

Qualifications

• Active Ohio Physical Therapist license.
• Experience in long-term care or geriatric rehab preferred.
• Strong clinical and communication skills.
• Ability to collaborate with interdisciplinary team.
• Experience with Net Health is a plus.

Responsabilités

• Evaluate residents to identify mobility limitations and functional impairments.
• Develop and implement individualized treatment plans for mobility, gait, strength, and pain management.
• Monitor progress and adjust plans as needed.
• Supervise and collaborate with PTAs.
• Educate residents and caregivers on plans and strategies.
• Maintain timely clinical documentation.
• Participate in care planning meetings.
• Follow clinical standards and facility policies.

What You'll Do

As a Physical Therapist (PT), you'll evaluate residents and develop individualized treatment plans to improve mobility, strength, balance, safety, and overall functional independence.

Responsibilities include:

• Evaluating residents to identify mobility limitations, strength deficits, balance concerns, and functional impairments
• Developing and implementing individualized treatment plans focused on mobility, gait training, strength, and pain management
• Monitoring resident progress and adjusting treatment plans as needed
• Supervising and collaborating with Physical Therapist Assistants (PTAs)
• Recommending assistive devices and adaptive equipment to promote safety and independence
• Educating residents, families, and caregivers on treatment plans and mobility strategies
• Maintaining timely and accurate clinical documentation
• Participating in interdisciplinary team meetings and care planning
• Following applicable clinical standards, regulatory requirements, and facility policies

What We're Looking For

• Active Ohio Physical Therapist (PT) license
• License in good standing with applicable licensing boards and regulatory agencies
• Long-term care, skilled nursing, or geriatric rehabilitation experience preferred, but not required
• Net Health documentation experience is a plus
• Strong clinical, communication, and decision-making skills
• Ability to collaborate effectively with residents, families, and the interdisciplinary care team
• Passion for helping residents improve or maintain their independence and quality of life
