Job summary

Qualifications

• B.S., M.S., or D.P.T degree from accredited physical therapy school.
• Current licensure in the state of applicable state
• CPR certification in Basic Life Support, Level C.

Responsibilities

• Adheres to hospital standards and policies, promoting a cooperative work environment and teamwork.
• Provides patient care assessment, functional evaluations, and treatment plans with goals.
• Maintains accurate documentation of evaluations, progress notes, discharge notes, and ICD-10 codes.
• Ensures legible documentation and timely data entry in patient records.
• Meets department productivity standards and adjusts schedules as needed.
• Participates in process management to improve quality and efficiency of services.
• Promotes professional development and education of other health care workers.
• Communicates effectively with patients, physicians, staff and external customers.
• Delivers age/developmentally appropriate care and stays current with best practices.

Education
