Job summary

Qualifications

• B.S., M.S., or D.P.T. degree in physical therapy from an accredited program.
• Current licensure in the applicable state.
• CPR/Basic Life Support (BLS) Level C certification.

Responsibilities

• Adheres to hospital standards to promote a cooperative work environment and follows policies.
• Provides patient care assessment and treatment with comprehensive evaluations and realistic goals.
• Maintains accurate documentation: evaluations, progress notes, discharge notes, and ICD-10 codes.
