Continuum Therapy Partners seeks a licensed Physical Therapist for a part-time daytime role at St. Francis Health Services in Milwaukee, WI. We champion a therapist-led approach and collaborative care within a growing national network.

Requirements include a Doctor of Physical Therapy (DPT), current Wisconsin PT license, and NPTE; 1 year of PT experience is preferred. You will evaluate and implement individualized treatment plans, document progress, and educate patients and families on home

• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities:

• Doctor of Physical Therapy (DPT) degree required
