Job summary

Continuum Therapy Partners, LLC is seeking a Part-time Physical Therapist for Meadow View Nursing Center in Berlin, PA. You will evaluate, develop, and implement individualized treatment plans, coordinate with other disciplines, and educate patients and families on home exercise programs while promoting injury prevention.

The position requires a DPT, current PA license, NPTE, and 1 year of experience is preferred.

Qualifications

• Doctor of Physical Therapy (DPT) degree required.
• Current, unencumbered state Physical Therapy License; must pass the NPTE.
• 1 year of previous experience as a Physical Therapist preferred.
• Evaluates, develops, and implements individualized treatment plans in coordination with other disciplines.
• Documents patient progress and adjusts plans of care based on clinical outcomes.
• Educates patients and families on home exercise programs and injury prevention.

Physical Therapist Requirements & Responsibilities
